Person-centred conversation prompts

A printable two-page worksheet for preparing a respectful discussion, listening carefully and recording questions without making assumptions.

Editorial preview prepared for Aayush to review. Original illustrative material, not a record of delivered client work or individual clinical advice.

Use this worksheet to prepare a general learning conversation. Start with the person's priorities, ask how they prefer to communicate and leave room for something you have not thought to ask. The prompts are not a formal assessment.

The PDF includes a preparation page and a review page with writing space. Use a fictional situation or anonymized learning notes only. Identifiable information belongs in the appropriate authorized service system.

Try it: choose one prompt, discuss how it could be phrased more accessibly, and note what you would check rather than assume. A completed sheet is a reflection aid, not an agreed support plan.
