Team reflection planner

A printable agenda and review sheet for separating observations, assumptions and one agreed learning question.

Editorial preview prepared for Aayush to review. Original illustrative material, not a record of delivered client work or individual clinical advice.

A short team reflection can become more useful when people know the question being explored. This worksheet offers a structure for a general learning discussion: describe an observation, identify an assumption, consider whose perspective is missing and agree what to review.

The PDF provides prompts and space for notes. It does not replace supervision, incident reporting, safeguarding processes or an individualized plan. It is not a form for uploading case information to this site.

Try it with the fictional handover example in Work. Write one observation without an interpretation, then choose one question to bring to an appropriate supervisor. Return to the sheet to record what you learned.
